Personal Information Form

Instructions:

1. Click on Download Personal Information Form. A new tab with the PDF File will open

2. Place the mouse cursor over a PDF file link, then click your right mouse button. A small menu will appear.

3. Select “Save As” from the menu. A location box appears.

4. Save the PDF file to a location you can remember. You can also save it is your Desktop.

5. Open the PDF file and fill in the form with your information.

6. Save the changes. Click on File, then Save As.

7. Change the name of the File (Preferably with your name). Click Save

8. You can now FAX us a copy or Email us your Information.
